Our patients pay a yearly retainer, which covers a comprehensive history and annual physical as well as ongoing, as-needed medical care. As there are certain procedures and services not covered under this plan, patients also need to retain primary insurance or Medicare.

Comprehensive history and annual physical

A one-hour comprehensive history and physical launches the doctor-patient relationship. Patients will provide pertinent old medical records, a thorough family history, and a three-day nutrition diary one week prior to the appointment. The one-hour physical includes:

Prostate and well-woman exams

Nutrition and fitness assessment

Vision and audio testing

Screening for anxiety and/or depression

Lab tests, to potentially include comprehensive metabolic panel (CMP), complete blood counts (CBC), thyroid function test (TSH) for ages 35+, fasting lipid panel (FLP), cholesterol (HDL and LDL), glucose tests, PSA prostate cancer screening for ages 40+, urinalysis (UA), Microalbumin-Creatinine Ratio for artery-lining health, oxygen saturation, resting EKG and EKG stress test, spirometry, among others deemed necessary

Screenings for bone density, ankle-brachial index (ABI) for vascular health in the legs

Update routine immunizations. Tetanus and influenza vaccines are covered.

If not up-to-date, we coordinate and schedule mammogram for ages 40+ and colonoscopy for ages 50+.

Medical Support Services

Ongoing medical support is included under your yearly retainer and includes:

24/7 access to your personal physician by email, text, Skype, FaceTime, and/or personal cellphone

Telephone consultations

Same day/next day appointments with minimal to no wait times in office

Extended, relaxed office visits

After-hours and weekend office visits, if matters require more than telephone conversation

House calls if warranted

Heart attack and stroke prevention program

Online wellness program, discounted gym membership

Hospital visits to act as your advocate with hospital staff and specialists; prompt in-clinic after-care

Minor laceration repair, minor skin biopsies, and cryotherapy for skin lesions

Prompt referral, coordination of care with a strong network of specialists, follow-up to discuss care plan

Assistance in coordinating care with physicians in other cities if required while you’re traveling

Discounted gym memberships of $36.95/month + tax are available at Life Changing Fitness, which also offers one-on-one personal training and nutrition coaching options.

Services and procedures not covered

Patients will need to retain private insurance or Medicare to cover the following, as they are not covered under your yearly retainer:

Hospitalizations and emergency room visits

Prescription drugs

Lab work not covered under annual physical; X-rays, and pap smears pathology evaluation

Mammograms, MRI, and CT Scans, Echoes, Stress Echoes, Nuclear Stress Tests

Specialist care, outpatient and surgical procedures, or medical services provided outside our office

Non-routine immunizations, such as shingles, hepatitis, pneumonia, or meningitis vaccines

Workers compensation

David W. Ball, M.D. received his medical degree from the University of Mississippi Medical Center. His internship and Internal Medicine residency was completed at the University of Tennessee in Chattanooga, TN.

 

His primary focus is functional fitness and wellness. He is the founder/CEO of 4-Ever Fit M.D., a science-based Wellness Company. He is also the co-founder of Tri-Fit, a non-profit organization that promotes wellness through exercise, nutrition, and accountability relationships. He enjoys mountain biking, weight lifting, scuba, working on his farm and spending time with his family.
